Register and Request Accommodations

While students can register with the Office of Accessibility Services (OAS) at any time and request accommodations, newly admitted students who are requesting accommodations are encouraged to register with OAS as early as possible after confirming enrollment.

How do students register and request accommodations?

  1. Students requesting reasonable accommodations for the first time must register with OAS by completing the .
  2. Please provide any relevant documentation regarding the referenced condition that rises to a level of disability from the appropriate treatment provider(s) via the OAS Disability Verification Form.
    • Healthcare providers can send information and completed Disability Verification Form by fax at 203-576-4601 to Âé¶¹´«Ã½, Office of Accessibility Services. Email for the Disability Verification Form.
    • Please note that a note from a doctor or mental health provider listing suggested accommodations with no explanation of history, test results, etc. will not be sufficient. Good examples of documentation list the condition, provide a thorough explanation of the history, duration and severity of the condition and may or may not include test results.     • Students are encouraged to meet with the Accessibility Services Manager even if they are still waiting for documentation or are not able to access it yet to help get them started with the process.

Accommodations expire at the end of each term. Returning students can request to renew accommodations by filling out the .

Confidentiality statement

Accessibility Services is responsible for receiving and maintaining disability-related documentation and information for students with disabilities attending Âé¶¹´«Ã½. All documentation in the student’s file is treated confidentially and will not be released to anyone not involved in the accommodation and service-delivery process with the following exceptions:

  1. the student gives OAS a signed release to share disability-related information with the person(s) or office(s) named on the release;
  2. OAS will release disability-related information as required and/or permitted by the law and/or a court order;
  3. the student threatens to harm themselves or others
  4. the student files a disability-related complaint, appeal, grievance, or lawsuit against any University office or employee(s).

Otherwise, College faculty and staff need to know only

  1. that the student has been through the disability documentation review process; and
  2. what accommodations have been approved by OAS to meet the student’s disability-related needs.
